IELTS for International Doctors

If you're an international medical graduate (IMG) hoping to practise in the UK, proving your English language proficiency is a non-negotiable requirement for GMC registration, PLAB, and most UK medical jobs. For many, the IELTS Academic test is the most accessible and widely accepted option—but it's not easy. 

Too often, candidates underestimate the difficulty of the exam, particularly the Writing section, and end up wasting time and money on multiple attempts. This guide is designed to help you prepare strategically and effectively for the IELTS, with a strong focus on Writing Task 2, where many IMGs lose marks. 

 

Understanding the IELTS Requirement for GMC and UK Work 

The Academic IELTS (either UKVI or non-UKVI version) is required if you are pursuing the PLAB route, applying for GMC registration, or applying for NHS roles. The minimum scores required in one sitting are: 

  • Overall: 7.5 

  • Each component (Listening, Reading, Writing, Speaking): 7.0 

Higher scores (7.5 in all components) may be needed for Foundation Programme (FY1) or Standalone FY2 applications. 

Alternatively, the OET (Occupational English Test) is accepted by the GMC and UK employers—but IELTS remains more widely available and often less expensive.

Among the four components of the IELTS, Writing Task 2 is the one most commonly failed. This part of the test requires a formal, academic essay of around 250 words, written in 40 minutes. You have 40 minutes for Task 2. Use it wisely:

  • 30 minutes to write 

  • 5 minutes to proofread 

Regular timed practice will improve both speed and structure.

Examiners look for grammatical variety. Incorporate: 

  • Simple, compound, and complex sentences 

  • Passive voice where appropriate 

  • Modal verbs (e.g., might, must) 

  • Conditional sentences (e.g., “If X had happened, Y would have followed.”) 

  • Relative clauses and rhetorical questions 

This showcases fluency and flexibility in your written English. 

8. Proofread Every Time 

Before submitting, read your essay with fresh eyes. Correct grammar, spelling, awkward phrasing, and ensure your argument flows logically. Even minor improvements can boost your score. 

9. Practise Regularly and Consistently 

The only way to get comfortable with IELTS Writing Task 2 is to write often. Work on essays across multiple topics and question types to improve adaptability and vocabulary. 

10. Seek Feedback 

Having someone review your writing—ideally an IELTS tutor or someone familiar with the exam—is crucial. They can identify blind spots and help you improve faster than self-study alone.

What If IELTS Isn’t the Right Fit for You? 

If IELTS is proving difficult or unavailable in your country, consider the OET Medicine test instead. It’s designed specifically for healthcare professionals and accepted by the GMC and UK Visas and Immigration. However, it's not accepted for FY1/Foundation entry and may be more expensive or less accessible. 

Some IMGs may also be eligible to prove English proficiency via: 

  • A medical degree taught and examined entirely in English (with strict criteria) 

  • Working in an English-speaking country for 2+ years 

  • Holding a UK job offer with language verification from the employer 

⚠️ If you’ve already failed IELTS or OET, you can no longer use these alternate options—you must pass one of the exams.
